This form is provided for all people who have taken the CERT class to update their personal information so you can be contacted quickly and efficiently in time of emergency, and to be able to advise you when refresher CERT courses are offered.
Most of the fields below are self explanatory, but here are some guidelines. If you are a member of a neighborhood CERT, enter the name of the CERT in "Group" - example "Old Mountain View". If you signed up to be a registered Disaster Service Worker at the end of your CERT class, select the YES button after DSW. If you have a Ham Radio license, enter your Ham call sign, otherwise leave it blank.
Your information is considered private, and is used only by the Mountain View Office Of Emergency Services for authorized activities. Thank you
